Tools to Individualize Clinical Decisions for HCPs in the Era of Co-morbidities & Survivorship

Due to effective medical care, there are growing number of survivors who suffered from a myocardial infarctions, cancer, or other life-threatening diseases. This group represents a new generation of patients that must manage their past conditions as a chronic condition.

It is important that GPs and FPs understand the specific care required for these patients as they are typically responsible for these patients’ long-term care. This session will characterize these patients and discuss how these patients should be best managed both from a medical and a psycho-social perspective.
